Affordable Quality Care

  • Discount Program - Access Community Health Centers offers medical, dental and behavioral health services on a sliding scale for patients who are uninsured with low incomes. All uninsured patients pay a minimum
    co-payment for their care at the time of their appointment. Any remaining charges will be billed to the patient.
  • Medicaid and BadgerCare - Our medical and dental clinics accept Medicaid and BadgerCare.
  • Medicare - Access Community Health Centers medical clinics accept Medicare. Because we are a Federally Qualified Community Health Center, Medicare will pay for some additional preventative services not normally covered by Medicare.
  • Private Health Insurance - Our clinics accept many private insurance plans.
  • Co- Payments - Patients with coverage that requires co-payments must pay that amount at the time that care is provided.

Discount Program

Access Community Health Centers offers medical, dental, and behavioral health services on a sliding scale for patients who are uninsured with low incomes. Our staff works with the patient to determine their eligibility for the Discount Program.

    How do I apply?
    To receive a discount on the cost of care, patients must complete an application and provide proof of income for themselves and for their spouse or domestic partner at the time of application. Applications are available at any clinic visit. Only one application is required for each family.

    What information do I need to complete the application?
    To receive a discount on the cost of care, patients' must complete an application and include information about their dependents. In addition, patients' are required to provide proof of income for themselves as well as their spouse or domestic partner at the time of application.

    How often do I apply?
    Most of our patients apply as part of their first visit, although applications are accepted at any time. Depending on the patient's employment status, applications should be completed once or twice yearly to make sure that the patient is receiving the maximum discount.

    Does each person in my family need to apply?
    One application may be completed for you, your spouse and your children under the age of 18 who are living in the household.

    How do I find out the amount of my discount?
    The amount of discount varies depending on the income of the patient. Patients find out the amount of their discount within two weeks of completing the discount program.
