Payment Options
Everyone is welcome at Access Community Health Centers. We are committed to providing affordable, quality medical and dental services to people from throughout Dane County.
Affordable Quality Care
- Medicaid and Badgercare: ACHC’s dental and medical clinics accept Medicaid and Badgercare.
- Medicare: ACHC’s medical clinic accepts Medicare. Because ACHC is a Federally Qualified Community Health Center, Medicare will pay for some additional preventative services not normally covered by Medicare.
- Private Health Insurance: Most private insurance is accepted at Access Community Health Centers.
- Uninsured: ACHC offers a Sliding Discount Program (see below). All uninsured patients must pay a co-payment fee for their care at the time of the office visit and be billed for any remaining charges.
- Co-Payments: Patients with coverage that requires co-payments must pay that amount at the time that care is provided.
Discount Program
Access Community Health Centers’ Sliding Discount Program is the way that ACHC discounts the cost of medical or dental care for those patients with incomes below 200 percent of the Federal Poverty guidelines. All eligible patients who apply for the Sliding Discount Program will need to provide proof of income.
How do I apply?
Applications are to be completed at the Health Center. Patients may complete an application at anytime. No formal appointment is needed. Patients wanting to receive the discount for a specific service date should complete the application no later than two months from that service date. Patients must provide proof of income for ACHC's Sliding Discount Program at the time the application is completed.
What information do I need to complete the application?
Patients must bring proof of income for each member of the household that is included on the application. The discount will not be applied without proof of income.
How often do I apply?
Applications must be completed once a year or when your income or family size changes significantly.
Does each person in my family need to apply?
One application may be completed for you, your spouse and your children under the age of 18 who are living in the household.
How do I find out the amount of my discount?
ACHC’s Financial Counselor will process your application within two weeks and inform you of your discount. You may receive up to two statements before the discount is reflected on your statement.
