Duties
This position is a combination role of Community Resource Specialist (CSR) and Health Benefits Counselor (HBC). The CRS’s main function is to provide case management to at-risk patients and coordinate necessary specialty referrals. This position also tracks and reports activities related to case management and referrals. The HBC role assists patients in pursuing enrollment in government programs such as Medical Assistance and BadgerCare. The HBC also assists under-insured patients with ACHC’s sliding discount program. This position communicates with patients regarding their financial obligation for ACHC’s services and negotiates and monitors patient payment agreements.
Required Skills
1. High school diploma or equivalent AND health care experience OR Associates Degree in health care, social services or related field
2. Fluent in Spanish and English.
3. Basic Computer Skills. Able to send and receive email and use basic computer programs such as MS Excel and MS Word.

Duties
This is a VOLUNTEER position through the Americorps program. The person hired will work at Access Community Health Centers in Madison, WI. The Community Resource Specialist position is essential to provide ACHC patients with comprehensive clinical care and ensure better health outcomes. The Community Resource Specialist collaborates with other clinical staff to assess patient needs and coordinate services. The main functions of the Community Resource Specialist is to provide case management to at-risk ACHC patients and coordinate necessary specialty referrals. The Community Resources Specialist is also responsible for tracking and reporting activities related to case management and referrals. Must have a thorough understanding and demonstrate the ability to find appropriate resources for patients with complex social issues such as domestic violence, poverty, chronic mental illness, disability, homelessness and substance abuse. This may be limited to making an internal referral for provision of this support by another discipline in the clinical setting.
Provide case management to ACHC patients – specifically prenatal, and otherwise at-risk patients.
Develop and maintain relationships with community organizations in order to facilitate medical and social service referrals, keep our information current, and represent our organization in a positive light.
Provide outreach and follow-up for newborns and children in need of well-child visits at ACHC.
Assist interdisciplinary team with crisis intervention, problem solving and idea sharing.
Facilitate enrollment and appointments for specialty and voucher diagnostic care. Manages tracking of utilization of resources. Works with other staff to ensure that diagnostic and consultative reports are back to the provider in a timely manner.
Works with agencies, providers and clinical staff to trouble shoot problems in communication regarding reports and of financial aspects of the voucher and specialty care programs.

LPN-MEDICAL ASSISTANT

Duties
Responsible for patient flow from intake to completion of patient visit. Assists with medical procedures, administers injections and immunizations under order of physician, and does written and oral communication required to support a medical practice.
